1- You must attach your examination request here via the form below or send it to us by fax at 514 281-0262.

2- Please bring your Medicare card on the day of the exam and make sure it is still valid. Please also bring your doctor's request.

3- Radiologie Varad Clinic generally operates on an appointment basis. To cancel an appointment, we ask that you contact us by phone or email at least 24 hours in advance. Otherwise, a penalty will apply ($50 for a RAMQ covered exam or $100 for a private exam).
